After you make your initial appointments, your intake forms will be emailed to you. All email communication is encrypted and HIPAA compliant to ensure your confidentiality. Once your forms are completed and signed, your therapist will receive them so he or she can be prepared to address your specific challenges.

Insurance Information

All of our licensed therapists are in-network providers with managed care, but not every clinician is in-network will every company. Your copay or coinsurance payment is expected at the time of service. For those therapists who do not participate with your insurance (or for couples therapy which is not covered by insurance), full payment by cash, check or credit card is expected at the time of service. If you are not in-netowrk but have Out-Of-Network benefits, please discuss this with our administrative staff when you make your initial appointment.
